If your child's birth is registered in Western Australia without the natural father's details, these particulars may be added to the registration by completing the "Add father's details in a child's birth" application form (See download below) and payment of a fee. Both parents must also provide appropriate identification.
When adding the natural father's particulars to the birth registration, you have the option of registering your child's family name as:

The Registry can only add the particulars of a child's natural father.
