Adding father's details

If your child's birth is registered in Western Australia without the natural father's details, these particulars may be added to the registration by completing the "Add father's details in a child's birth" application form (See download below) and payment of a fee. Both parents must also provide appropriate identification.

When adding the natural father's particulars to the birth registration, you have the option of registering your child's family name as:

the mother's family name
the father's family name
a combination of the mother's and father's family name in any order with or without a hyphen; or any other family name.

Note:

The Registry can only add the particulars of a child's natural father.

If your child was born interstate, contact the Registry Office in the State or Territory where your child was born.

We are located at Level 10, 141 St Georges Terrace, Perth. The public counter is open Monday to Friday 8.30am - 4.30pm (except public holidays).

Note: Overseas applicants must pay in Australian dollars using an international money order or bank draft, which should be made payable to: Registrar of Births, Deaths and Marriages.
