The Western Australian Registry of Births, Deaths & Marriages issues commemorative certificates for births registered in Western Australia. If the birth occurred outside WA, contact the relevant Registry Office. Certificates will only be issued where:
On payment of a fee and subject to eligibility, you can apply for a commemorative birth certificate in person at the Perth Registry Office. An application form can be completed when you apply at the Registry or you can print an online application form and bring it with you (see downloads below). Also bring the appropriate original identification with you.
A person must be at least 16 years of age to apply for a certificate and must provide their identification.

If you are not an eligible applicant you must provide the written consent or authority from an eligible person and you must also provide identification for yourself and the person for whom you are acting. (See Letter of Authority download below).
There is unrestricted access and no identification requirements are needed for births that occurred more than 75 years ago.
We are located at Level 10, 141 St Georges Terrace, Perth. The public counter is open Monday to Friday 8.30am - 4.30pm.
Payment is accepted by:
Cheques or money orders should be made payable to: Registrar of Births, Deaths & Marriages.
If you want your document to be posted by "Express Post" an additional fee of $4 is payable for postage within Australia or $10 for international postage.
