Correct a birth certificate

If information on a certificate issued by the WA Registry of Births, Deaths & Marriages is incorrect or incomplete, you should contact the Registry. Where the error was made by the Registry, no fee is payable to correct the certificate and any certificates issued will be replaced free of charge.

However, if incorrect or incomplete information was provided to the Registry, then a correction fee is payable. This fee includes the issue of a replacement certificate. Any request to correct certificate information must be supported by documentary evidence which confirms the correct details.

Identification requirements

To correct Registry records, applicants need to supply identification to verify they are entitled to correct the registration. You must provide original identification if applying in person at the Registry. If applying by post, clear photocopied identification documents must be posted with your application. Do not post original documents.

Where do I apply?

We are located at Level 10, 141 St Georges Terrace, Perth.  The public counter is open Monday to Friday 8.30am - 4.30pm. (Except Public Holidays)

Payment may be sent by:

Cheques or money orders should be made payable to: Registrar of Births, Deaths & Marriages.

Note: Overseas applicants must pay in Australian dollars using an international money order or bank draft, which should be made payable to the "Registrar of Births, Deaths and Marriages".

Delivery Times

If all necessary supporting information, including identification documentation, is provided and the required fee is paid, we will amend a record within four days plus postal delivery time.
