How do I apply?

An application form can be completed when you come to the court or you can print an online application form and bring it with you (see downloads below). Also bring the appropriate original identification with you.

Certificates will only be issued where:

  • you are eligible to receive the certificate
  • suitable identification is provided
  • fees are paid.

Where do I apply?

All Courts can fax applications for death certificates for the period 1841 onwards to the Perth Registry Office for processing. Selected Courts (see below) have online facilities and are able to produce death certificates from 1940 onwards usually on the same day.
