Perth Registry Office

How do I apply?

An application form can be completed when you come to the Registry or you can print an online application form and bring it with you (see downloads below). Also bring the appropriate original identification with you.

Certificates will only be issued where:

  • you are eligible to receive the certificate
  • suitable identification is provided
  • fees are paid.

Where do I apply?

We are located at Level 10, 141 St Georges Terrace, Perth. The public counter is open Monday to Friday 8:30am to 4:30pm (except public holidays).

To avoid the usual busy times please consider visiting the Perth Registry between 8:30am to 10am or 2pm to 4pm.


Payment is accepted by:

  • Cash, cheque or money order
  • Credit card (MasterCard or Visa only)
  • Eftpos (debit card).

Cheques or money orders should be made payable to: Registrar of Births, Deaths & Marriages.

If you want your document to be posted by Express Post an additional charge of $6 is payable for postage within Australia or $15 for international postage.
