An application form can be completed when you apply at the Registry or you can print an online application form and bring it with you (see downloads below). Also bring the appropriate original identification with you.
Certificates will only be issued where:
We are located at Level 10, 141 St Georges Terrace, Perth. The public counter is open Monday to Friday 8.30am - 4.30pm (except public holidays).
To avoid the usual busy times please consider visiting the Perth Registry between 8.30am - 10am or 2pm - 4pm.
Payment is accepted by:
Cheques or money orders should be made payable to: Registrar of Births, Deaths & Marriages.
If you want your document to be posted by Express Post an additional charge of $5 is payable for postage within Australia or $13 for international postage.
