Replacement Change of Name Certificate

If your change of name is registered in Western Australia from 31 January 1957 and you would like a replacement certificate, you can apply for one at the Registry.

Identification requirements

If applying for a certificate in person your original identification documents must be provided. If applying by post, clear, photocopied identification documents must be posted with your application. Do not post original documents.

Eligibility

All certificate applications are also subject to the Registry's Change of Name Certificate Access Policy.
